29 CFR 1918.22(a): Whenever practicable, a gangway of not less than 20 inches (.51 m) in width, of adequate strength, maintained in safe repair and safely secured was not used. This violation was discovered on December 19, 2012 on long liner vessel SI-FC-990 at the starkist Samoa dock located in Atu'u, American Samoa; where an employee was exposed to a drowning hazard when loading the vessel with food supplies, without using a gangway.
