29 CFR 1910.23(c)(1): Every open-sided floor or platform 4 feet or more above adjacent floor or ground level shall be guarded by a standard railing (or the equivalent as specified in paragraph (e)(3) of this section) on all open sides except where there is entrance to a ramp, stairway, or fixed ladder:    On or about January 3, 2013, at the Hoist Line, employees required to perform dipping and coating operations were exposed to a fall hazard at the loading area platform(s) of approximately 5 feet above ground level.  Platforms were not guarded by a standard railing (or the equivalent as specified in paragraph (e)(3) of this section) on all open sides except where there was an entrance to a ramp, stairway, or fixed ladder.

29 CFR 1910.124(h)(4):  For employees who work with chromic acid, periodic examinations of their exposed body parts, especially their nostrils must be provided:    On or about January 3, 2013, at the Hoist and Hand Lines, employees required to perform dipping and coating operations with chromic acid solution were not provided with periodic examinations of their exposed body parts, especially their nostrils.

29 CFR 1910.134(c)(2)(ii): The employer must establish and implement those elements of a written respiratory protection program necessary to ensure that any employee using a respirator voluntarily is medically able to use that respirator, and that the respirator is cleaned, stored, and maintained so that its use does not present a health hazard to the user. Exception: Employers are not required to include in a written respiratory protection program those employees whose only use of respirators involves the voluntary use of filtering facepieces (dust masks):    On or about January 3, 2013, in the facility, employees required to perform dipping and coating operations were provided and used half-mask air-purifying respirators voluntarily.  The employer did not establish and implement those elements of a written respiratory protection program, including medical evaluation and training, to ensure that the respirator itself did not present a health hazard to the user.

29 CFR 1910.1200(f)(5)(i):  The employer shall ensure that each container of hazardous chemicals in the workplace is labeled, tagged or marked with the identity of the hazardous chemical(s) contained therein and appropriate hazard warning:    On or about January 3, 2013, at the Hoist and Hand Lines, employees required to perform dipping and coating operations were exposed to the hazard of contact with hazardous chemicals including acids, alkali and metals.  The employer did not ensure that each process tank was labeled, tagged or marked with the identity of the hazardous chemical(s) contained therein and appropriate hazard warning.
