29 CFR 1910.28(b)(1)(i): Except as provided elsewhere in this section 29 CFR 1910.28, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i): a. On or about August 26, 2025, in the King Kong rack plating department, an employee removing chain hoist attachments from a bridge crane at heights over fifteen (15) feet above the ground, was not protected from falls, thereby exposing the employee to fall hazards.

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a. On or about August 26, 2025, at the above worksite, the employer did not develop machine specific procedures for controlling hazardous energy for the Uesco Crane, four ton, Serial #17-4297 bridge crane located on the King Kong Rack Plating line, thereby exposing employees to hazards associated with unexpected re-energization of equipment.
