29 CFR 1910.303(b)(7)(i): Unused openings in boxes, raceways, auxiliary gutters, cabinets, equipment cases, or housings were not effectively closed to afford protection substantially equivalent to the wall of the equipment: (a) On or about January 10, 2013, the inspection revealed an opening in the HMI breaker 16 circuit breaker panel that was not effectively closed. The circuit breaker panel was located in the SE corner of the main gate guard shack. (b) On or about January 14, 2013, employees were potentially exposed to serious injury from electrical hazards due to a knockout opening that was not effectively closed. The opening was located on the left side of circuit breaker panel #281205, located in the PAD shop of Building 80, next to the table for cutting insulation. (c) On or about January 14, 2013, employees were potentially exposed to serious injury from electrical hazards due to a knockout opening that was not effectively closed on the right side of the 125 -260 volt white switch box. The switch box was located in building 80 PAD shop, near circuit breaker panel #281205 and the insulation cutting table. (d) On or about January 14, 2013, employees were potentially exposed to serious injury from electrical hazards due to unused openings that were not effectively closed on the bottoms of four 440 volt switches. The switches were located in Building 50 PCI warehouse, NW corner.

29 CFR 1910.333(a)(1): Live parts to which an employee was exposed were not deenergized before the employee worked on or near them: Building 63: On or about January 10, 2013, and times prior thereto, qualified electricians were allowed to work on energized breaker panels with out deenergizing the panel before working on it. This exposed employees to electrical arc flash and shock hazards, from working in the area of live equipment.

29 CFR 1910.24(h): Standard railings were not provided on the open sides of all exposed stairways and stair platforms: On or about January 14, 2013 the inspection revealed a staircase railing that did not have a midrail. The staircase was located at the PCI warehouse in Building 50 at 5555 N Channel Ave in Portland, Oregon.
