29 CFR 1910.212(a)(5):     Fan blade guard(s) were not provided where the periphery of the blades was less than seven feet from the floor or working level:    (a) On or about January 14, 2013, employees were potentially exposed to serious injury from a ventilation fan that did not have a blade guard. The fan was located on the SE side of the upper platform of the water treatment plant, at 5555 N. Channel Ave in Portland, Oregon.
