(a) On or about January 14, 2013 employees were potentially exposed to serious injury from the auto switch P5 button for the rainwater sump that was damaged and missing the button cover. The button was located on the control panel in the tank farm control room at 5555 N Channel Ave in Portland, Oregon.

29 CFR 1910.303(b)(7)(i): Unused openings in boxes, raceways, auxiliary gutters, cabinets, equipment cases, or housings shall be effectively closed to afford protection substantially equivalent to the wall of the equipment. (a) On or about January 14, 2013 employees were potentially exposed to serious injury from electrical hazards due to a junction box with a missing knockout. The junction box was part of the rainwater sump pump system and was located by seperator 4 in the tank farm at 5555 N Channel Ave in Portland, OR.
