29 CFR 1910.219(c)(3): Vertical or inclined shafting seven feet or less from floor or working platform was not enclosed with stationary casing(s): (a) On or about January 14, 2013 employees were potentially exposed to serious injury from the unguarded inclined shaft at mix tank #1 in the NW corner of the upper level at the water treatment plant at 5555 N Channel Avenue in Portland, Oregon. (b) On or about January 14, 2013 employees were potentially exposed to serious injury from the unguarded inclined shaft at the clay tank at the first level of water treatment plant at 5555 N Channel Avenue in Portland, Oregon.
