29 CFR 1910.23(a)(8):     Every floor hole into which persons can accidentally walk shall be guarded by either:    In the Pressure Washer Area, a pressure washer employee was cleaning metal racks and was exposed to a trip and fall hazard from two troughs (3 1/2 inches wide, 1 1/2 inches deep and five feet in length) in the concrete to serve as drainage for the soapy water used in the presssure washing operation.

29 CFR 1910.133(a)(3):     The employer shall ensure that each affected employee who wears prescription lenses while engaged in operations that involve eye hazards wears eye protection that incorporates the prescription in its design, or wears eye protection that can be worn over the prescription lenses without disturbing the proper position of the prescription lenses or the protective lenses.    In the Assembly Area, an employee was exposed flying metal chips while using a portable power drill to countersink holes and cleaning with compressed air by wearing prescription glasses which do not incorporate eye protection in its design.

29 CFR 1910.242(b):     Compressed air used for cleaning. Compressed air shall not be used for cleaning purposes except where reduced to less than 30 p.s.i. and then only with effective chip guarding and personal protective equipment.    In the Assembly Area, an employee was using a compressed air nozzle to clean off dirt and debris from the metal racks that measured 70 psi when deadended.

29 CFR 1910.147(c)(1):     Energy control program. The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.    The company had not developed nor implemented an energy control program consisting of energy control procedures,training and periodic inspections.

29 CFR 1910.212(a)(1):     Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.    In the Production Area, an employee was exposed to an amputation hazard from the 15 inches of unused portion of the saw blade of the Hyd-Mech 18 Horizontal Saw Machine while cutting round and hollow metal stock to length.

29 CFR 1910.305(b)(1)(ii):     Unused openings in cabinets, boxes, and fittings shall be effectively closed.    In the Paint Area, a painter was exposed to an electric shock hazard from a metal cover that was missing for a 240 Volt AC electrical transformer box located and tied into the electrical disconnect switchbox to the Paint Booth.

29 CFR 1910.334(a)(1):     Portable electric equipment was not handled in a manner which would not cause damage:    In the Assembly Area, an employee was exposed to an electric shock hazard while using a portable power drill connected to an extension cord set that was plugged into a two receptacle outlet located on a post and strung across an aisleway and subject to damage from powered industrial trucks.
