29 CFR 1910.107(b)(5)(iv):     Space within the spray booth on the downstream and upstream sides of filters shall be protected with approved automatic sprinklers.      a)  The spray booth located in the assembly area of the facility where Catagory 4 flammable liquids were sprayed did not have automatic sprinklers installed in both the upstream and downstream side of the filters.

29 CFR 1910.1000(a)(2):  Employee(s) were exposed to an airborne concentration of Carbon Monoxide listed in Table Z-1 in excess of 50 ppm as an 8-hour Time Weighted Average concentration:     a)  On January 25, 2013 an employee operating fork lift #4 was exposed to an 8-hour time weighted average concentration of 50 ppm of Carbon Monoxide which exceeded the permissible exposure limit by 135% during a sample period of 444 minutes. Zero exposure was used for the unsampled portion of the shift.

29 CFR 1910.1200(f)(6)(ii):      Except as provided in 29 CFR 1910.1200(f)(7) and 29 CFR 1910.1200(f)(8), the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals and which, in conjunction with the other information immediately available to employees under the hazard communication program, would provide employees with the specific information regarding the physical and health hazards of the hazardous chemical;    a)  The cut off 1/2 gallon milk jug being stored adjacent to the paint booth on top of a 55 gallon barrel was not labeled, tagged or marked with the product identified and hazard of the chemical contained there iin (Finish 1 - paint thinner).
