29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level. (a.) On or about February 5, 2013, employees working from the side brackets extended from the top level of a supported scaffold were exposed to a 24-foot fall hazard where guardrails or personal fall arrest was not used.
