29 CFR 1910.95(g)(6): At least annually after obtaining the baseline audiogram, the employer did not obtain a new audiogram for each employee exposed at or above an 8-hour time-weighted average of 85 decibels : American Synthetic Fiber, LLC - on or about March 14, 2013, employees working along production lines and at the bailer were exposed to continuous noise levels exceeding 50% of the allowable 8-hour time-weighted average (greater than 85 dBA 8-hour TWA). The employer did not ensure that annual audiometric tests were performed for all employees exposed. An employee was exposed to 70.53% of the allowable 8-hour TWA sound level (equivalent sound level of 87.4 dBA). Sampling was performed for 492 minutes. An employee was exposed to 74.11% of the allowable 8-hour TWA sound level (equivalent sound level of 87.8 dBA). Sampling was performed for 478 minutes, with zero exposure assumed for the unsampled period of time of 2 minutes.

29 CFR 1910.95(k)(3)(ii): The employer did not ensure that each employee was informed of the purpose of hearing protectors, the advantages, disadvantages, and attenuation of various types, and instructions on selection, fitting, use and care: American Synthetic Fiber, LLC - on or about March 14, 2013, employees were exposed to sound levels above the Action Level of 85 dBA and were required to wear hearing protection. Several employees did not wear the hearing protection correctly, and did not demonstrate proper use when asked.
