On or about 12/9/14, at the Pendergrass facility, employees were exposed to unexpected energy start up as each authorized employee was not required to attach a lock to energies when servicing/maintaining/cleaning equipment and instead two employees would use a single lock when locking out equipment for maintenance purposes.
