29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D):  312 South Holland Drive, Pendergrass, GA (Throughout Plant) - On or about February 27, 2020, Employees were exposed to sudden energization or start-up of multi-energy source equipment/machinery such as, but not limited to Fare 1 machine. The company's written lockout/tagout specific procedures were found with the following deficiencies:       a. The energy control procedures did not specifically outline the steps for placement of locks on the multi-          energy source disconnect apparatus.         b. The energy control procedures did not specifically outline the steps for the removal of locks and restoring the           equipment back to service.                                                                                                                                                                                                                                                                                        c. Specific requirements for testing the machine or equipment to determine and verify the effectiveness of lockout          devices, tagout devices and other energy control measures were not included in the specific procedures.
