At the back wall of suite 132, on or about February 20, 2013, eye protection was not worn while using a Bosch 11255VSR BULLDOG Xtreme Rotary Hammer to drill holes in the concrete floor.

29 CFR 1926.403(i)(2)(i):     Live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by cabinets or other forms of enclosures, nor by any of the means listed in subparagraphs (A) through (D) of this paragraph.    At the back wall of suite 132, on or about February 20, 2013, the panel box was missing its dead front cover to prevent employees from accidental contact with live electrical parts.

At the back wall of suite 132, on or about February 20, 2013, there was no assured equipment grounding conductor program or ground-fault circuit-interrupter program when an extension cord was used to supply power to a Bosch 11255VSR BULLDOG Xtreme Rotary Hammer.  Employee used the drill to penetrate the concrete floor.

Employee used the drill to penetrate the concrete floor.

29 CFR 1926.405(g)(2)(iv):     Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:    At the project on or about February 20, 2013, a yellow 50 foot yellow extension cord's outer insulation was not secured to the plug.  The cord supplied power to a 110 VAC 110 VAC Bosch 11255VSR BULLDOG Xtreme Rotary Hammer.  Employee used the drill to penetrate the concrete floor.
