29 CFR 1926.417(b): Equipment or circuits that were deenergized were not rendered inoperative and did not have tags attached at all points where such equipment or circuits can be energized: (a) In the storage area electrical panel on or about February 21, 2013, and at times prior thereto, several 110 VAC circuit breakers, labeled for lights, were in the off position, but they were not rendered inoperable by removing the wires from the breakers, and they did not have tags. Some flexible cords hung from the ceilings with bare wires exposed about four feet from the floor. The wires in the electrical box and outlet, located in a demolished concrete wall, were energized. The walls were demolished with electric jack hammers.
