29 CFR 1926.403(b)(2): Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification:  In the warehouse area, electrical equipment was not installed as designed to be installed as the wires that protruded from the metallic sheathed cable were stripped back and inserted into a 110 VAC receptacle in an electrical box mounted to a wood stand.  The wires powered a 110 VAC fluorescent light that rested against the floor and a wall.  Employees were exposed to the hazard of contacting electicity.

29 CFR 1926.404(b)(1)(i): The employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites.  These requirements are in addition to any other requirements for equipment grounding conductors:  In the warehouse area, there was no ground-fault circuit-interrupters or assured equipment grounding conductor program to protect employees that used a 110 VAC drill to install a suspended ceiling from a scaffold platform.  The drill was powered through a flexible cord that was plugged into a temporay outlet on a wood stand.  The test button on the outlet would not trip the ground-fault circuit-interrupter in the outlet. Employees were exposed to the hazard of contacting electricity.

29 CFR 1926.451(f)(15): Ladders shall not be used on scaffolds to increase the working level height of employees, except on large area scaffolds where employers have satisfied the criteria in 29 CFR 1926.451(f)(15)(i)-(iv):  In the warehouse area, a 4 foot high step ladder was used on a mobile scaffold platform that was 4 feet wide by 6 feet long by 6 feet high.  Employees that used the ladder to install the suspended ceiling about two days prior to the OSHA inspection were exposed to the hazad of falling from heights that varied from about 7 to 8 feet to the concrete floor below.

29 CFR 1926.405(a)(2)(ii)(E): All lamps for general illumination were not protected from accidental contact or breakage:  In the warehouse area, two four foot long fluorescent light bulbs were not protected from accidental contact or breakage.  One end of the light fixture rested on the floor and the other end rested against the wall.  There was no cover or screen protecting the bulbs.  Employees that worked in the area with ladders, scaffolds, and other tools were exposed to the hazard of contacting electricity.
