The employees were installing sheathing on the roof. This exposed the employees to fall hazards. The violation was observed on or about April 16, 2013.

The employees used the ladder to climb, approximately 10-15 above the ground, to install sheathing on the roof. This exposed the employees to fall hazards. The violation was observed on or about April 16, 2013.

The ladder was on an unstable, unleveled surface, and was not secured. The employees were using the ladder to access the dormer, located approximately 10-15 feet above the ground. The employees were installing sheathing on the roof and dormer areas. This exposed the employees to a fall hazards. The violation was observed on or about April 16, 2013.
