29 CFR 1926.95(a): Personal protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, or maintained in a sanitary and reliable condition it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation, or physical contact.  At the front of the project site on May 9, 2013, appropriate hand protection (butyl or nitrile) was not used when an employee mixed and applied Hanil Portland. Employee was exposed to wet cement contacting the skin. The warning label on the bag stated "Be noted not to touch on skin."

29 CFR 1926.701(b): All protruding reinforcing steel, onto and/or into which employees could fall or come against, was not guarded to eliminate the hazard of impalement:   At the front of the project site on May 9, 2013, there were no rebar caps on numerous rebar that protruded about three feet from the foundation; where employees were working.

29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous:  At the project on May 9, 2013, employees were exposed to electrical shock hazards while using an ungrounded extension cord to power a Ridgid 1/2 inch, Spade Handle Mud Mixer, model number R71211, serial number CS12135D253818.  Xiang Wang Construction Corporation was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 314656877, citation number 1, item number 2 and was affirmed as a final order on August 19, 2010 with respect to a workplace located at the Sears building project, upper Tumon.
