Employee was observed working on a non-compliant elevated work platform supported and elevated by a Pettibone Model 8042 Rough Terrain Forklift with no fall protection in place.  Employee was utilizing stacked 2x6 boards as a platform to perform framing operations.  Condition observed on or about May 10, 2013.  Five Star Quality Construction Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.503(a)(1), which was contained in OSHA inspection number 699658, citation number 1, item number 2a and was affirmed as a final order on 4/22/2013, with respect to a workplace located at 1815 Lombard Street, Philadelphia, PA.
