29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:    (a) 1115 Lancaster Avenue Columbia, PA - Employee was utilizing a Genie S-60 aerial lift to perform work activities and was not protected/restrained inside the basket by the use of an attached lanyard, thereby exposing the employee to a possible fall hazard of approximately 28 feet, on or about May 31, 2013.    BRECKNONK BUILDERS LLC., WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD CFR 1926.453(b)(2)(v), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 312486418, CITATION NUMBER 2 ITEM NUMBER 1 AND WAS AFFIRMED AS A FINAL ORDER ON FEBRUARY 27, 2009 WITH RESPECT TO A WORKPLACE LOCATED AT 901 SOUTH MEDIA LINE ROAD, MEDIA PA, 19063.      No abatement certification or documentation required.
