29 CFR 1910.137(b)(2)(viii): Electrical protective equipment was not subjected to periodic electrical tests. Test voltages and the maximum intervals between tests shall be in accordance with Table I-5 and Table I-6. a. Boomerang Rubber, Inc. located in Botkins, Ohio: On or about June 5, 2013 the employer did not ensure a pair of rubber insulated gloves used to perform electrical work, were tested annually. The gloves used by a maintenance employee to perform troubleshooting in the Stat Refining building were last inspected on August 6, 2010. Boomerang rubber, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.137(b)(2)(viii) which was contained in OSHA inspection number 107944, citation number 1, item number 7 and was affirmed as a final order on January 10, 2013, with respect to a workplace located at 105 Dinsmore St., Botkins, Ohio 45306.

Boomerang Rubber, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.335(a)(1)(iv), which was contained in OSHA inspection number311604953, citation number 1, item number 12b which was affirmed as a final order on November 19, 2008 and OSHA inspection number 107944, citation number 3 item number 3b which was affirmed as a final order on January 10, 2013 with respect to a workplace located at 105 Dinsmore St., Botkins, Ohio 45306.
