29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D):  a. On or about December 22, 2018, the employer did not ensure that energy control procedure(s) for the Chop Line identified specific steps to follow to render the hydraulic and mechanical energy sources safe.  The procedure(s) lacked specific steps for blocking the baler head and controlling energy to the conveyors.  Employees were exposed to hazards, such as pinch points and rotating parts, when servicing the equipment.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:  a. On or about December 22, 2018, a periodic inspection of the energy control procedures was not performed to assure that the lockout/tagout procedure and requirements of 29 CFR 1910.147 were being followed.  The employer did not ensure that the energy control procedure for Chop Line equipment such as, but not limited to, the baler and conveyors was inspected.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a. On or about December 22, 2018, the employer did not ensure that baler #2 was adequately guarded to protect employees from pinch point hazards.  Employees who operated the equipment were exposed to pinch points created between the baler's housing and moving parts of the baler head and pushout cylinder.    b. On or about January 2, 2019, the employer did not ensure that conveyor belt #6 was guarded to protect employees from laceration and/or contusion injuries.  Employees that passed under the conveyor belt to reach work areas were exposed to being struck by the conveyor's cleats.

29 CFR 1910.147(c)(5)(ii)(D): Lockout devices and tagout devices did not indicate the identity of the employee applying the device(s):  a. On or about January 2, 2019, the employer did not ensure that lockout devices were marked to identify the employee applying the lock.  A lock without identifying information was used to lockout conveyors during a time an employee performed cleaning operations.    b. On or about December 29, 2018, the employer did not ensure that lockout devices were marked to identify the employee applying the lock.  A lock without identifying information was used during a time an employee cleared a jam at a chopper on the Chop Line.
