29 CFR 1910.147(d)(5)(i):All potentially hazardous stored or residual energy was not relieved, disconnected, restrained or otherwise rendered safe after the application of lockout or tagout devices to energy isolating devices: a) Boomerang Rubber, Inc., located at 105 Dinsmore Street, Botkins, OH 45306: On or about July 9, 2021, the employer did not ensure the application of lockout devices to energy isolating devices on the press #7 conveyor prior to an employee performing belt alignment and adjustment tasks. An employee performing these servicing tasks in an energized state at or near the nip point on conveyor #7 was exposed to amputation hazards.
