29 CFR 1910.37(b)(2): Each exit was not clearly visible and marked by a sign reading "Exit": Six Rivers National Forest, 1330 Bayshore Way, Eureka, CA: On June 25, 2013, and at times prior thereto, doors designated as exits on the facility's evacuation map were not clearly marked with an Exit sign. Employees were exposed to serious injuries associated with limited safe egress from the facility during fires and other types of emergencies.

29 CFR 1910.36(g)(2): Exit access(es) were not at least 28 inches (71.1 cm) wide at all points: a) Six Rivers National Forest, 1330 Bayshore Way, Eureka, CA, Administrative building, top floor: On June 25, 2013, and at times prior thereto, the employer obstructed the exit access by placing large cases and containers next to the emergency exit door. The exit access was restricted to approximately 18 inches exposing workers to serious injuries in the event of an emergency requiring safe egress from the building.
