29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition:   Six Rivers National Forest, Salyer Guard/Work Center: On or about June 26, 2013, and at times prior thereto, the workshop area was not kept orderly and was obstructed with multiple boxes, furniture, and other debris on the ground.  This condition exposed workers to slips, trips, falls and fire exposures from ignitable combustible materials not properly stored.  It also restricted worker egress during an emergency.

29 CFR 1910.37(b)(2): Each exit was not clearly visible and marked by a sign reading "Exit":   Six Rivers National Forest, Salyer Guard Station: On or about June 26, 2013, and at times prior thereto, exit doors in the workshop and throughout the facility were not clearly marked with an Exit sign.  Employees were exposed to serious injuries associated with delayed safe egress during fires and other emergencies including smoke inhalation and burns.

29 CFR 1910.157(c)(1):  Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries:    Six Rivers National Forest, Salyer Guard/Work Center: On June 26, 2013,  and at times prior thereto, portable fire extinguishers in the throughout the facilities were blocked, not properly mounted, and not readily accessible.  The workshop and warehouse had no working extinguishers in an area with a high number of combustible materials.  Employees were exposed to serious fire injuries including smoke inhalation and burns in the event of an emergency.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:   Six Rivers National Forest. Salyer Guard/Station, Repair Shop: On or about June 26, 2013, and at times prior thereto, the workshops compressor guard had been removed during serving and not replaced. This hazard exposed employees to the rotating equipments wheel and belt. Workers were exposed to serious amputation and pinch point hazards while operating or working near the equipment.

29 CFR 1910.303(f)(2):  Each service, feeder, and branch circuit, at its disconnecting means or overcurrent device, was not legibly marked to indicate its purpose, nor located and arranged so the purpose was evident:     Six Rivers National Forest, Salyer Guard/Work Center, Work Shop: On or about June 26, 2013, and at times prior thereto,  each branch circuit on the facility's main breaker panel was not marked to indicate its purpose or location. Employees were exposed to electrical and mechanical hazards during emergencies and other situations requiring the shut off of the correct circuits and/or equipment.    Inaccurate labeling of circuit breakers in the electrical cabinet hinder and delay an employees' ability to identify and shut down the appropriate circuit breakers during an electrical or equipment emergency.  Delays in shutting down electrical equipment increase the potential for serious injuries to employees during an emergency.

29 CFR 1910.304(g)(5)::The path to ground from circuits, equipment, and enclosures was not permanent, continuous and effective.    Six Rivers National Forest, Salyer Guard/Work Center: On June 26, 2013,  and at times prior thereto, the electrical wall outlet near the entrance to the warehouse was not properly grounded. Employees were exposed to serious electrical and fire hazards when operating power equipment off the wall outlet.

29 CFR 1910.303(g)(2)(i):     Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i):  exposing workers to accidental electrical shock hazards.     a) Six Rivers National Forest, Salyer Guard/Work Center: On June 26, 2013, and at times prior thereto, the main power breaker panel had an opening allowing accidental contact to the energized conductors and exposing employees to serious electrical injuries and shock.
