29 CFR 1910.36(g)(2): Exit access(es) were not at least 28 inches (71.1 cm) wide at all points:     Six Rivers National Forest, Lower Trinity Work Center, CA, Workshop: On June 26, 2013, and at times prior thereto, the employer obstructed the exit access by placing large cabinets, boards, and a standing fan next to the emergency exit door.  The exit access was restricted to approximately 17 inches; exposing workers to serious injuries in the event of an emergency requiring safe egress from the building.

29 CFR 1910.37(b)(2): Each exit was not clearly visible and marked by a sign reading "Exit":   Six Rivers National Forest, Lower Trinity Work Center, repair shop: On or about June 26, 2013, and at times prior thereto, exit doors throughout the facility were not clearly marked with Exit signs.  Employees were exposed to serious hazards associated with safe egress during fires and other emergencies.

29 CFR 1910.303(f)(2):  Each service, feeder, and branch circuit, at its disconnecting means or overcurrent device, was not legibly marked to indicate its purpose, nor located and arranged so the purpose was evident:      Six Rivers National Forest, Oak Bottom Fire Station: On or about June 26, 2013, and at times prior thereto, each branch circuit on the facility's main breaker panel was not marked to indicate its purpose or location. Employees were exposed to electrical and mechanical hazards during emergencies and other situations requiring the shut off of the correct circuits and/or equipment.   Inaccurate labeling of circuit breakers in the electrical cabinet hinder and delay an employees' ability to identify and shut down the appropriate circuit breakers during an electrical or equipment emergency.  Delays in shutting down electrical equipment increase the potential for serious injuries to employees during an emergency.

29 CFR 1910.23(e)(2): Stair rails did not consist of a top rail, intermediate rail and/or posts, with the vertical height not more than 34 inches (86.36 cm) nor less than 30 inches (76.2 cm) from upper surface of top rail to surface of tread in line with face of riser at forward edge of tread:    Six Rivers National Forest, Lower Trinity, CA: On June 26, 2013, and at times prior thereto, the stair railing on the stairs leading from the ground floor of the workshop to the upper floor storage room was constructed without a mid rail.  Workers were exposed to serious injuries from slips and falls when using the stairwell for accessing the upper level storage room.

29 CFR 1910.213(h)(1): The sides of the lower exposed portion of the blade of radial saw(s) were not guarded to the full diameter of the blade by a device that automatically adjusted itself to the thickness of the stock and remained in contact with stock being cut to give maximum protection possible for the operation being performed:  a) Six Rivers National Forest. Lower Trinity Camp Station, Repair Shop: On or about June 26, 2013, and at times prior thereto, the Dewalt power retracting wood saw had a defective chain type guard that did not properly guard the saw blade.  Workers were exposed to serious cut and amputation hazards while operating the equipment.
