29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes (including skylights) by covers: (a). Worksite located at 6001 NW 63rd Street, Building #3, Kansas City, Missouri- Employees were exposed to fall hazards in that employees had cut floor holes opening in a walking/working area in order to access one level to other levels. The floor opening was left uncovered.
