(b) Scaffold cross bracing were serving as guardrails.  (c)  A fixed ladder was attached to a scaffold system greater than three tiers high (15 to 18 feet).   (d)  Internal scaffold stairway systems are mandatory on any scaffold higher than three tiers (15 to 18 feet).
