29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes by covers:   (a) On or about December 3rd, 2015, at a job site located at 1300 Baltimore Ave. in Kansas City, MO, employees were exposed to trip and fall hazards in that floor holes existed on multiple stair landings within each of the four stair towers at the site.  Although not large enough for an employee to fall through, the multiple floor holes did expose employees to potential foot, ankle, and leg injuries from tripping and falling.   Abatement certification is required for this Item.  Use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.  NOTE:  IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.  FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1,000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
