29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  Jobsite located at 4121 Plum Tree, Wichita, KS; employees using the portable extension ladder to access the roof were exposed to fall hazards of up to 11 feet 2 inches from the roof and/or ladder in that the side rails of the ladder extended approximately 1 foot above the landing surface of the roof.

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards:  Jobsite located at 4121 Plum Tree, Wichita, KS; employees using ladders were not trained to recognize and minimize hazards associated with ladder use.  Employees used the portable extension ladder with the side rails of the ladder extended only 1 foot above the landing surface of the roof.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   Jobsite located at 4121 Plum Tree, Wichita, KS: employees engaged in residential roofing work on an 8 in 12 pitch roof were exposed to fall hazards of up to 11 feet, 2 inches from the eave to the ground.  Fall protection equipment available at the job site was not being used.  Jaime Hernandez was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 935675, citation number 01, Item number 01, and was affirmed as a final order on 11/07/13, with respect to a workplace located at 2655 Fountain, Wichita, KS 67220.
