29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: At the jobsite located at 1500 E Main St, Marion, KS; employees using a portable extension ladder to access the roof were exposed to fall hazards of up to eight (8) feet from the roof and/or ladder in that the side rails of the ladder extended approximately 1 foot above the landing surface of the roof. Jaime Hernandez was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 949370, citation number 1, item number 1 and was affirmed as a final order on 12/24/2013, with respect to a workplace located at 4121 Plum Tree, Wichita, KS.

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards: Jobsite located at 1500 E Main St, Marion, KS; employees using ladders were not trained to recognize and minimize hazards associated with ladder use. Employees used the portable extension ladder with the side rails of the ladder extended approximately 1 foot above the landing surface of the roof. Jaime Hernandez was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 949370, citation number 1, item number 2 and was affirmed as a final order on 12/24/2013, with respect to a workplace located at 4121 Plum Tree, Wichita, KS.
