29 CFR 1903.19(c)(1): The employer did not certify to OSHA, within 10 calendar days after the abatement date, that the cited violation had been abated. a) The employer failed to provide documentation and certification for abatement citation 1 item 1 in which an employee was exposed to flying debris while cutting plywood sheathing with a portable circular saw. The employee was not wearing eye protection. On or about December 12, 2013. b) The employer failed to provide documentation and certification for abatement citation 1 item 2 in which the ladder used by employees to access the roof did not extend at least 3ft above the landing. On or about December 12, 2013. c) The employer failed to provide documentation and certification for abatement citation 1 item 3 in which an employee was observed climbing an extension ladder while carrying plywood roof sheathing. The employee did not have at least one free hand to grasp the ladder. On or about December 12, 2013. d) The employer failed to provide documentation and certification for abatement citation 1 item 4 in which an employee was observed climbing an extension ladder while carrying plywood roof sheathing. The employee had to use two hands to carry the plywood, exposing him to potential fall hazards. On or about December 12, 2013. e) The employer failed to provide documentation and certification for abatement citation 1 item 5 in which the employer did not ensure that each employee using ladders was trained in the recognition and mitigation of fall hazards associated with ladders.
