Facilities Maintenance: On or about 03/03/14, an unguarded wall opening was not provided with a guardrail while employees installed an HVAC unit as a result employees were exposed to a 9 feet fall.

29 CFR 1910.335(a)(1)(ii): Protective equipment shall be maintained in a safe, reliable condition and shall be periodically inspected or tested, as required by 1910.137. a.) Crane Maintenance: On or about 03/13/14, the Class 2, Type 1 rubber-insulting gloves used when testing live electrical equipment were not provided with an electrical test every 6 months. Note: Testing is required before first issue and every 6 months thereafter. If the insulating equipment has been electrically tested but not issued for service, it may not be placed into service unless it has been electrically tested within the previous 12 months.
