29 CFR  1917.45(d)(2): The employer used a crane or derrick having a visible or known defect affecting safe operation:  a)  Norfolk International Terminal Crane #8, Navious Indigo Vessel: On or about August 01, 2021, a ship to shore crane hoist brake was made inoperative and the crane put back into service after the hoist brake caused the crane to emergency stop, as a result employees supporting the cargo move were potentially exposed to a struck by hazard.  No abatement certification or documentation required.
