29 CFR 1917.116(c): An elevator or escalator with defects which could affect safety was used: a) Virginia International Gateway Terminal: On or about 01/21/2021, the electrical wiring to the passenger compartment top door on elevator #2 was removed as a result employees were exposed to possible injuries. b) Virginia International Gateway Terminal: On or about 01/21/2021, the passenger compartment top door safety feature on elevator #3 did not function as designed therefore employees were exposed to possible injuries. c) Virginia International Gateway Terminal: On or about 01/21/2021, the passenger compartment top door safety feature on elevator #4 did not function as designed therefore employees were exposed to possible injuries. d) Virginia International Gateway Terminal: On or about 01/21/2021, the foot pedal in elevator #1 used in activating the Speed limiter device was disconnected as a result employees were exposed to possible injuries. e) Virginia International Gateway Terminal: On or about 01/21/2021, the foot pedal cable in elevator #3 used in activating the Speed limiter device was stretched as a result employees were exposed to possible injuries. f) Virginia International Gateway Terminal: On or about 01/21/2021, the foot pedal in elevator #5 used in activating the Speed limiter device did not function as designed therefore employees were exposed to possible injuries. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1917.116(e): Thorough inspections of elevators and escalators were conducted at intervals exceeding one year.: a) Virginia International Gateway Terminal: On or about 01/21/2020, the employer failed to conduct a thorough inspect of the ZPMC TKC elevators 1-6, in that inspections required by the manufacturer were not conducted. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement. 29 CFR 1917.116(e): Records of the results of the latest annual elevator inspections were not posted in the elevators: a) Virginia International Gateway Terminal: On or about 01/21/2020, the employer failed to post in elevators 1, 2, 3, 4 and 6 the latest annual elevator inspection. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
