29 CFR 1926.501(b)(3): Each employee in a hoist area was not protected from falling 6 feet or more to lower levels by guardrail systems or personal fall arrest systems: (a) Building 11, second level: On or about March 12, 2014, and at times prior, workers without personal fall protection were exposed to falling 11 feet 1 inch while pulling stock from the hoist area while no guard rail was installed. (b) Building 11, second level: On or about March 12, 2014, and at times prior, a worker without personal fall protection was exposed to falling 12 feet 9 inches while standing on top of the material pile with no effective top rail. Abatement Note: Abatement certification and documentation are required for this item (see enclosed Certification of Corrective Action Worksheet).

29 CFR 1926.501(b)(14): Each employee working on, at, above, or near wall openings (including those with chutes attached), where the outside bottom edge of the wall opening was 6 feet (1.8 m) or more above lower levels and the inside bottom edge of the wall opening is less than 39 inches (1.0 M) above the walking/working surface, is not protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system: (a) Building 11, second level: On or about March 12, 2014, and at times prior, workers without personal fall protection were exposed to falling 12 feet 8 inches while working near a 33 inch wide wall opening with no top rail that was 25 inches from the subfloor. Abatement Note: Abatement certification and documentation are required for this item (see enclosed Certification of Corrective Action Worksheet).
