29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502: (a) Skytor Building Group, Inc. at 4380 W 29th Ave, Denver, CO 80212: On or before May 26, 2016, the exposing employer did not ensure the use of fall protection when cutting drainage holes on the roofs of two 2-story multi-family homes at the construction site. This condition exposed employees to a fall in excess of 18 feet.

29 CFR 1926.503(a)(1): The employer shall provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards. (a) Skytor Building Group, Inc. at 4380 W 29th Ave, Denver, CO 80212: On or before May 26, 2016, the exposing employer did not ensure that each employee engaged in roofing operations was trained to recognize the hazards of working at elevation and trained in the use of fall protection. This condition exposed employees to a fall in excess of 18 feet.
