29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:       (a)  Skytor Building Group, Inc. at 4162 Pecos, Denver, CO 80211: On or about November 11th, 2016, the exposing employer did not ensure the use of fall protection for employees stick framing the mechanical rooms on the third story roofs. This condition exposed employees to a fall hazard of approximately 8 feet.        (b)  On or about November 11th, 2016, the exposing employer did not ensure the use of fall protection for employees transitioning between building rooftops. As a result, employees were observed jumping from a 3rd story roof to the detached building roof 3 feet away.  This condition exposed employees to a fall hazard of approximately 25 feet.       (c)   On or about November 11th, 2016, the exposing employer did not ensure the use of fall protection for employees using solid wood planking to transition between detached buildings. This condition exposed employees to a fall hazard of approximately 25 feet.     Skytor Building Group, Inc. was previously cited for a violation of this Occupational Safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 1150661, citation number 1, item number 1, issued on 7/29/2016, with respect to a workplace located at 4380 W. 29th Avenue, Denver, CO.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:   (a)  Skytor Building Group, Inc. at 4162 Pecos, Denver, CO 80211: On and prior to November 12, 2016, the exposing employer did not ensure each employee engaged in various wood stick framing activities was trained to recognize the hazards of working at elevations and trained in the use of acceptable fall protection. This condition exposed employees to observed fall hazards up to an estimated 25 feet.   Abatement Note: Abatement Note: The employer shall assure that each employee has been trained, as necessary, by a competent person qualified in the following areas:   a) The nature of fall hazards in the work area;   b) The correct procedures for erecting, maintaining, disassembling, and inspecting the fall protection systems to be used;   c) The use and operation of guardrail systems, personal fall arrest systems, safety net systems, warning line systems, safety monitoring systems, controlled access zones, and other protection to be used;   d) The role of each employee in the safety monitoring system when this system is used;   e) The limitations on the use of mechanical equipment during the performance of roofing work on low-sloped roofs;   f) The correct procedures for the handling and storage of equipment and materials and the erection of overhead protection; and   g) The role of employees in fall protection plans; and   h) The standards contained in this subpart.   Upon completion of fall protection training, the employer must verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or signature of the employer.   Skytor Building Group, Inc. was previously cited for a violation of this Occupational Safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 1150661, citation number 1, item number 1b, issued on 7/29/2016, with respect to a workplace located at 4380 W. 29th Avenue, Denver, CO.
