29 CFR 1903.19(c)(1): The employer did not certify to OSHA, within 10 calendar days after the abatement date, that the cited violation had been abated.  a) The employer failed to provide documentation and certification for abatement of citation 1 item 1 in which employee was observed working from the raised forks of a rough terrain fork lift (RTFL), installing sheathing with a pneumatic nail gun and hammer. An employee on the ground below was not wearing a hard hat and was thus exposed to an overhead struck-by hazard from potential falling tools and/or plywood sheathing. On or about 06/17/14.  b) The employer failed to provide documentation and certification for abatement of citation 1 item 2 in which the employees was not wearing eye protection when operating equipment. Observed on or about 06/17/14.  c) The employer failed to provide documentation and certification for abatement of citation 1 item 3 in which employees was observed cutting plywood sheathing with a Makita circular saw.  The saw's power cord was damaged and the conductors were exposed, thus exposing the employee to electrical shock hazards. Observed on or about 06/17/14.

Observed on or about 06/17/14.  b) The employer failed to provide documentation and certification for abatement of citation 1 item 5 in which employees was observed working from the raised forks of a rough terrain fork lift (RTFL), using an unsecured pallet as his walking working platform.  The employee was not using fall protection and was thus exposed to a fall hazard of approximately 16 feet. Observed on or about 06/17/14.

The saw's power cord was damaged and the conductors were exposed, thus exposing the employee to electrical shock hazards.     b) The Waves, South Little Rock Ave., Ventnor:  On or about 04/3/2014, a WVM Construction, Inc. employee was observed cutting plywood sheathing with a Makita circular saw.  The extension cord powering the saw was damaged and the conductors were exposed, thus exposing the employee to electrical shock hazards.
