29 CFR 1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.9(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent:  On or about April 18, 2014, the employer did not institute an effective hearing conservation program for employees exposed to noise levels in excess of the allowable 8-hour Time Weighted Average (TWA) action level of 85 dBA.      a:  Dosimetry performed on a production employee for 459 minutes indicated that the employee was exposed to a 72.99% noise dose which is equivalent to approximately 87.73 dBA.      b:  Dosimetry performed on a machine operator for 477 minutes indicated that the employee was exposed to a 85.36% dose which is equivalent to approximately 88.86 dBA.      Elements of a hearing conservation program include, but are not limited to, sound level monitoring, audiometric testing on all affected employees, hearing protection, training, and record keeping.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure   On or about April 18, 2014, flexible cords were used as permanent wiring, exposing employees an electrical hazard.                                                              a:  An extension cord was used as permanent wiring and affixed to the structure to power a 110 volt fan near the slitter.  The cord was run from an outlet, up a beam, and down to where the fan was used.     b:  An extension cord in the munchy room was used as permanent wiring and affixed to the structure to power a 220 volt sump pump.  The cord was ran from an outlet and along a wall to the sump pump.

29 CFR 1910.305(j)(2)(i): No receptacles may be installed so as to require an energized attachment plug as its source of supply:  On or about April 18, 2014, a flexible cable was cut and used to power a receptacle box which, in turn, powered a water cooler and UV bug light, exposing employees to an electrical hazard.
