29 CFR 1910.147(c)(5)(ii)(D): Lockout devices and tagout devices did not indicate the identity of the employee applying the device: a) Delstar Technologies, Inc., 601 Industrial Road, Middletown, DE 19709 (Winder): On or about August 12,2022, lock out tags and locks were used on a group lockout tag out device that did not indicate the identity of the employee applying the device. Four (4) lockout/tag out devices were suspended from a disconnect switch controlling 480 V to the laminator drive cabinet.
