29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:     On or about April 28, 2014, inside the house, at the job site, the employer did not ensure employees exposed to falling object hazards wore hard hats to protect themselves.  Employees performing construction activities including framing 2 x 4 inch wood studs and using electric saws were observed working 10 feet below other employees framing rafters without the use of hard hats for protection. This practice created a condition for employee exposure to struck by hazards from overhead falling objects that could strike employees on the head.

29 CFR 1926.102(a)(1):     Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:     The ARMANDO IBARRA CONSTRUCTION was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 940617, citation number 1, item number 1 and was affirmed as a final order on 12/31/2013, with respect to a workplace located at 200 Marie Jeanne, North Little Rock, AR 72113.     On or about April 28, 2014, on the ceiling joists, at the job site, the employer did not ensure that employees using nail guns were protected by the use of protective eyewear.  The employees actively used the nail gun on the day of this inspection while stick constructing the roof rafters with 2 x 8 inch studs on the dwelling.  This condition exposed the employee to serious eye injuries.

29 CFR 1926.501(b)(13):     Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b).     The ARMANDO IBARRA CONSTRUCTION was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 940617, citation number 1, item number 2 and was affirmed as a final order on 12/31/2013, with respect to a workplace located at 200 Marie Jeanne, North Little Rock, AR 72113.    On or about April 28, 2014, on ceiling joists, at the job site, the employer did not ensure that each employee working and walking on wood-framing were protected from falling greater than 6 feet by a fall protection system.  Alternative fall protection system use was not attempted.  Employees were stick constructing the 2 x 8 inch roof rafters on a 2,188 square foot residential dwelling.
