29 CFR 1926.25(a): During the course of construction, alteration, or repairs, debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures: On or about March 8, 2018 and times prior thereto, at the worksite located at 2013 Wilson Drive in Little Rock, Arkansas, the work area was not kept clear from debris and other items. Employees were observed walking over loose lumber and other items that predominated the walking areas used by employees. This condition exposed employees to trip hazards.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): On or about March 8, 2018 and times prior thereto, at the worksite located at or near 2013 Wilson Drive in Little Rock, Arkansas, employees were observed installing roof decking greater than six feet above the lower level without any type of fall protection systems. This condition exposes employees to fall from heights hazards.
