Remick Pkwy., Lockport , NY:  Employees  engaged in construction work including roofing & siding work  were not trained to recognize potential hazards on this type of jobsite,  including but not limited to falls from roofs, ladders, skylights,  stairs & scaffolds, electrical hazards, unguarded tools/machines, fire hazards, and the need for personal protective equipment to protect them from various hazards. They  also were not trained in the applicable OSHA regulations to control or eliminate these hazards.    ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.404(b)(1)(i): Employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section, or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites:    a)  On or about 05/05/14, on  roof of the  apartment building at 83 E. Remick Pkwy., Lockport , NY:    A circular saw was used by an employee to cut plywood. The saw was connected to 120 volt ac power via an extension cord which was plugged into a receptacle on the back side of the building. No GFCI or assured equipment grounding program was being used.    NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means:    a) On or about 05/05/14, on the back side of apartment building at 83 E. Remick Pkwy., Lockport , NY:   An employee was standing on a ladder and was using a pitchfork and his bare hands to remove old siding from the around  the electrical service drop  to the building. The wires were not de-energized  and were not insulated and the voltage was approx.  177 to 240 volts-ac.      NO ABATEMENT CERTIFICATION REQUIRED

Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:   a)  On or about 05/05/14, on  roof of the  apartment building at 83 E. Remick Pkwy., Lockport , NY:  One employee engaged in roofing work was exposed to a fall of up to 18 ft.  because the siderails of an extension ladder used to access the roof only extended approx.  8 inches above the edge of the  roof.
