29 CFR 1926.451(b)(5)(ii): The end of a platform greater than 10 feet in length extended over its support more than 18 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees without tipping, or has guardrails which block employee access to the cantilivered end:  a) On or about 01/15/20, on a roofing job site, at a house at 230 East Ave.,Lockport, NY: A ladder jack scaffold consisting of 2 support ladders, an access ladder, and an approx. 20 ft. long aluminum pik was erected at the northwest corner of the house and was used by employees to install roofing shingles. The pik extended over the bracket on the north support ladder by approx. 7 feet, and over the bracket on the south support ladder by approx. 6 feet.  NO ABATEMENT DOCUMENTATION REQUIRED         D & M Home Improvement Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 1411625, citation number 1, item number 1, and was affirmed as a final order on 10/10/2019, with respect to a workplace located at 28 Ivy Lea, Tonawanda, NY 14223.

29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:   a) On or about 01/15/20, on a roofing job site, at a house at 230 East Ave.,Lockport, NY: A ladder jack scaffold consisting of 2 support ladders, an access ladder, and an approx. 20 ft. long aluminum pik was erected at the northwest corner of the house. Three employees were on the scaffold and were installing roofing shingles. The employees were exposed to a fall of approx. 18 ft. to the ground because no fall protection system was in place.  NO ABATEMENT DOCUMENTATION REQUIRED        DM Home Improvement Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 1234454, citation number 1, item number 2, and was affirmed as a final order on 10/12/2017, with respect to a workplace located at 33 Sterling Place, Lancaster, NY 14086.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) On or about 01/15/20, on a roofing job site, at a house at 230 East Ave.,Lockport, NY: The side rails of the extension ladder set up on the ground on the east side of the house, and used by employees to access the steep upper roof were only about 18 inches above the steep roof. b) On or about 01/15/20, on a roofing job site, at a house at 230 East Ave.,Lockport, NY: The side rails of the extension ladder set up on the ground on the east side of the house, and used by employees to access the steep dormer roof above the front entrance, were only about 4 inches above the roof. c) On or about 01/15/20, on a roofing job site, at a house at 230 East Ave.,Lockport, NY: The side rails of the extension ladder set up on the peak of the dormer roof above the front entrance , and used by employees to access the high steep dormer roof above, were only about 4 inches above the roof. d) On or about 01/15/20, on a roofing job site, at a house at 230 East Ave.,Lockport, NY:  The side rails of the extension ladder set up on the ground at the southwest corner of the house, and used by employees to access the steep upper roof were only about 12 inches above the roof. e) On or about 01/15/20, on a roofing job site, at a house at 230 East Ave.,Lockport, NY:  The side rails of the extension ladder set up on the ground at the northwest corner of the house, and used by employees to access the Aluminum pik on a ladder jack scaffold were only about 12 inches above the pik.  NO ABATEMENT DOCUMENTATION REQUIRED       D & M Home Improvement Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 1411625, citation number 2, item number 2, and was affirmed as a final order on 10/10/2019, with respect to a workplace located at 28 Ivy Lea, Tonawanda, NY 14223.   DM Home Improvement Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 1234454, citation number 1, item number 4, and was affirmed as a final order on 10/12/2017, with respect to a workplace located at 33 Sterling Place, Lancaster, NY 14086.

Pitch of the roof was approx. 10 in 12.   b) On or about 01/15/20, on a roofing job site, at a house at 230 East Ave.,Lockport, NY: An employee engaged in roofing work on the steep main high roof on the west side of the  house was exposed to a fall of approx. 20 ft. because he was not using fall protection. Pitch of the roof was approx. 10 in 12.    c) On or about 01/15/20, on a roofing job site, at a house at 230 East Ave.,Lockport, NY: An employee engaged in roofing work on the steep roof on the north side of the dormer on the west side of the house was exposed to a fall of approx. 20 ft. because he was not using fall protection. Pitch of the roof was approx. 10 in 12.    d) On or about 01/15/20, on a roofing job site, at a house at 230 East Ave.,Lockport, NY: An employee engaged in roofing work on the steep porch roof on the back of the house was exposed to a fall of approx. 10 ft. because he was not using fall protection.
