29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(b) or (c): (a) AGL Construction Company at 4200 9th Ave, Denver, CO 80220: On and before May 14, 2014, the employer did not ensure that each employee conducting work in an excavation was protected from cave-in, in that the protective system was inadequate because the top of the protective shield was 31 inches below grade. This condition exposed employees to a crushing hazard by excavation wall cave-in. AGL Construction Company was previously cited for a violation of the same or substantially similar condition of this Occupational Safety and Health Standard 29 CFR 1926.652(a)(1) which was contained in OSHA Inspection Number 314661687, Citation Number 1, Item 1b issued on December 14, 2010 and became Final Order on January 14, 2011 with respect to a workplace located at W. 60th Ave. and Sheridan Blvd., Arvada, CO 80003.
