29 CFR 1926.405(a)(2)(ii)(I): Flexible cords and cables used for temporary wiring were not protected from damage: (a) On or about February 5, 2018, employees were exposed to shock and other electrical hazards from the extension cord that was connected to a generator and powered the DeWalt masonry saw, that was strung across the path of construction traffic and was run over by vehicles including a personnel cart and a fully loaded wheel loader.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws: (a) On or about February 5, 2018, employees were exposed to shock and other electrical hazards from the extension cord that powered the DeWalt masonry saw, because the cord insulation was not connected to the plug, thus placing strain directly on the live conductors.
