29 CFR 1926.25(a): During the course of construction, alteration, or repairs, form and scrap lumber with protruding nails, and all other debris, shall be kept cleared from work areas, passageways, and stairs, in and around buildings or other structures. a. On May 28, 2014, front of the job site, a pile of scrap lumber with protruding nails, loose lumber, broken drywall and other debris was not kept clear from around the work areas exposing workers to tripping and fall hazards.

On May 28, 2014, the electrical extension cord used to provide power to the portable Mikita chipping gun and circular saw did not have ground fault circuit interrupter protection.
