29 CFR  1926.403(b)(2): Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification:  In the SandCastle Theater Seating Area:  A Ryobi Miter saw used to cut lumber for concrete forms was powered by a connection to a relocatable power tap  connected in series with an extension cord to the power source.  Employees were exposed to electrical and fire hazards.

29 CFR  1926.451(b)(1)(i): Each platform unit (e.g., scaffold plank, fabricated plank, fabricated deck, or fabricated platform) was not installed so that the space between adjacent units and the space between the platform and the uprights were no more than 1 inch (2.5 cm) wide, except where the employer can demonstrate that a wider space is necessary.  In the outdoor café area: The scaffold built along the side of the building, used to install formwork for the external staircase, was not fully planked on the first, second, and third levels. Employees were exposed to fall hazards.

29 CFR 1926.701(b): All protruding reinforcing steel, onto and/or into which employees could fall or come against, was not guarded to eliminate the hazard of impalement:  In the SandCastle Theater Seating Area: Rebar that protruded from the walking/working surface around the formwork for the stadium seating was not guarded. Employees were exposed to impalement hazards.  Jeida Guam was previously cited for a  violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.701(b), which was contained in OSHA inspection number 1590031, citation number 2 item number 1, and was affirmed final order on July 25, 2022 with respect to  a workplace located on Punzalan Street in Barrigada, Guam.  Jeida Guam was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.701(b), which was contained in OSHA inspection number 1287049, citation number 1, item number 2 and was affirmed as a final order on March 26, 2018, with respect to a workplace located at Naval Base Guam, EODMU-5 (Adjacent to Building 2112A SNDL), Santa Rita, Guam, 96915.

Employees were exposed to fall hazards.
