29 CFR 1926.501(b)(4): Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes:  a) 1535 Cecil B. Moore Ave, Philadelphia, PA  The employer did not ensure that each employee was protected from falling through the hole in the floor on the walking/working surface by installing some type of fall protection system. This condition exposed the employees to a fall hazard.This condition was observed on or about May 8, 2014.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:  a)  1535 Cecil B. Moore Ave, Philadelphia, PA  The employer did not ensure that each employee on the residential construction site was protected from falling approximately 10 feet to the lower levels by using or installing some type of fall protection system around the perimeter of the structure. This condition was observed on or about May 8, 2014.

29 CFR 1926.20(b)(1): The employer did not initiate and maintain an accident prevention program as may be necessary to comply with the General safety and health provisions covered in 29 CFR 1926.20:  a) 1535 Cecil B. Moore Ave, Philadelphia, PA  - The employer did not maintain a safety/health program, thus exposing the employees to workplace hazards, including but not limited to fall hazards greater than 6 feet. This violation was observed on or about May 8, 2014.  Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

Moore Ave, Philadelphia, PA  - Employees were not instructed in the recognition and avoidance of unsafe conditions and the regulations to their work environment to control or eliminate hazards. The employees were exposed to fall hazards greater than 6 feet. This violation was observed on or about April 11, 2014.  Abatement certification and documentation required within 10 days after abatement date.
